New Jersey Reentry Corporation (NJRC) is an employment-focused, data-driven, non-profit organization serving individuals returning from state prison, county jail, addiction treatment centers, and veterans of the theater of combat. The NJRC Care Navigation Center in Hackensack, New Jersey, connects court-involved individuals, veterans, and those battling a substance use disorder with integrated addiction and mental health services, collaborating with hospitals, agencies, and academic institutions. The Center oversees a peer recovery coach training program and hosts an Annual Summit to enhance treatment pathways for New Jersey’s Reentry population and ensure “best practices.”

NJRC is currently seeking a Care Navigator with relevant work experience. This is a full-time salaried position with a full benefits package, Monday through Friday.

  1. Summary of Position
    • Respond to all incoming calls from NJRC Sites and Preferred Providers on a daily basis to ensure warm handoffs to substance use disorder and mental health treatment, ultimately securing appointments prior to NJRC program participants leaving initial intake.
    • Arrange care in between care, working closely with Preferred Provider liaisons and program participants, ensuring the highest quality of customer service.
    • Obtain releases of information from the program participants and the preferred provider to ensure continuity of care throughout the continuum.
    • Work diligently with Preferred Providers and program participants to improve access to care by addressing social determinants of health.
    • Hold Preferred Providers accountable for sending biweekly progress reports and certificates of completion to track program participant outcomes.
    • Provide comprehensive care management, care coordination, seamless transitional care, and act as the gatekeeper for all NJRC program participants throughout the state.
    • Assures program goals and objectives are accomplished by fulfilling job and team responsibilities, as assigned.

  • Accurately and on a timely basis completes tasks required by the Agency’s time collection system

III. Essential Duties and Responsibilities - Includes the following, and other duties may be assigned:

  • Answers all incoming calls from NJRC sites, program participants, and Preferred Providers
  • Works with Program Director to facilitate Care Navigation Center Trainings for all NJRC sites and Preferred Providers
  • Responsible for securing treatment appointments throughout the continuum in real time
  • Fulfills case management services, including, but not limited to, coordinating or directly facilitating linkages to services, service coordination, treatment admission assistance, and so forth
  • Increases access to care by working closely with Preferred Providers by addressing social determinants of health
  • Maintains meticulous documentation in Salesforce, ensuring that all linkages, appointment outcomes, treatment compliance, testimonials, and program participant satisfaction metrics are met
  • Individualize program participant care by being assigned to cases on a daily basis that are navigated by the Care Navigator
  • Participates in daily caseload assignment meetings and meets weekly to review with the Program Director
  • Provides input into the formulation of program participant-centered care navigation plans in collaboration with Preferred Providers (that serve as indicators for evaluating progress) and realistic time frames for achievement; performs reviews of progress and evaluates the effectiveness of program participant achievements and preferred providers' performance

V. Age-Specific Competencies - For Adult Population:

  • Working knowledge of Levels of Care for Substance Use Disorder and Mental Health Treatment
  • Ability to build professional relationships with/ preferred providers
  • Ability to increase program participant engagement through excellent communication and relationship building throughout their recovery journey
  • Ability to navigate through complex social determinants of health by having knowledge specifically pertaining to barriers that include, but are not limited to, transportation, housing, legal, and vocational needs

Education/Experience:

  • Bachelor’s degree and/or Master's in Social Work, Psychology, Counseling, or related field from an accredited college or university preferred. Candidate can also have a CADC or be in pursuit of CADC with a high school diploma with/ 1-3yrs of experience working with the substance use disorder/ Mental Health population.

Language Ability:

  • Ability to read, analyze, and interpret general business periodicals, professional journals, technical procedures, or governmental regulations.
  • Ability to write reports and business correspondence.
  • Ability to effectively present information and respond to questions from groups of managers, consumers, and the general public.
  • Ability to speak and understand the program participant’s primary language

Salary:

Starting salary is $50,000. Negotiable
