The Care Navigator coordinates and carries out outreach efforts and provides case management services and referrals to external resources for individuals and families impacted by substance use disorders (SUD). This role focuses on reducing barriers to long-term recovery through coordination of care and linkage to essential services such as housing, childcare, transportation, employment, education, and other basic needs. Working collaboratively with clinical staff, patient support teams, and community partners, the Care Navigator works with the patient to identify barriers and unmet needs, facilitates interventions, and offers wrap-around support to help patients achieve stability and sustained recovery.

as the Care Navigator you will:

  • Collaborate with clinical staff, interdisciplinary teams, and community partners to identify and address patient and family needs, including medical, behavioral health, and social service supports.
  • Conduct engagement specialist phone calls to patients to promote treatment participation, increase motivation for recovery, and streamline access to necessary agency supports and resources.
  • Develop and implement individualized care coordination plans to remove barriers to treatment, enhance stability, and support positive outcomes.
  • Provide direct support and referrals to community resources for things like childcare, housing, employment and transportation.
  • Coordinate care for patients identified across multiple systems, including hospitals, shelters, justice settings, community agencies, and internal referrals to ensure continuity and quality of care.
  • Respond promptly to referrals and crisis situations by assessing needs, stabilizing acute circumstances, and linking individuals to appropriate services.
  • Engage in community outreach and education efforts, providing harm reduction strategies and information about SUD and SUD treatment to patients, families, and support networks.
  • Build and maintain strong partnerships with community organizations, healthcare providers, and other stakeholders to enhance collaboration and resource access.
  • Maintain accurate documentation of all interactions, referrals, and outcomes in the electronic health record in accordance with program, regulatory, and grant requirements.
  • Participate in staff meetings, case consultations, and professional development activities to support quality improvement and consistent service delivery.
  • Screen patients to identify unmet needs and determine grant eligibility; completes grant enrollment and manages ongoing purchases in accordance with grant requirements to support patient needs.
Requirements:

Qualified candidates will have a bachelor’s degree in a human services field and one year of related work experience. This position requires a valid driver's license and the ability to pass an extensive background check.

Salary and Benefits

ASAC offers competitive compensation, pay starts at $22.71 with additional compensation for those with extensive experience.
